Affordable Care Act (ACA) – Notice of Availability of Form 1095-C
DAV Thrift Stores has elected to furnish Form 1095-C for the 2025 calendar year by making it available upon request, as permitted by IRS Notice 2025-15.
Form 1095-C contains important information about the health coverage offered to you by your employer for the 2025 tax year.
This form is not required to be filed with your federal income tax return, but you may keep it for your records.
When Forms Will Be Available
Forms will be available beginning February 1, 2026.
How to Request Your Form 1095-C
If you would like to receive a copy of your 2025 Form 1095-C, you may request it at any time by contacting:
